Role: Wellbeing Worker – Living Well Support Service (Pennine)

Salary: £23,690 FTE

Hours:

  • Post 1: 21 hours per week, worked over a 6-week rota, alternating 3 and 4 days, 6-hour shifts between 9am–7pm (including some weekends and bank holidays)
  • Post 2: 24 hours per week, worked over a 6-week rota, 4 days per week, 6-hour shifts between 9am–7pm (including some weekends and bank holidays)

About the Role

Are you looking for a varied and rewarding role where you can make a difference in people’s lives every day? We are recruiting two Wellbeing Workers to join our Living Well Support Service team across the Pennine areas.

Age UK Lancashire’s Living Well Support Service helps people aged 18+ regain independence after a hospital stay or illness. We support hospital discharges, reduce unnecessary admissions, and help people remain at home rather than entering residential care. In the past year alone, we have supported over 9,000 people across the county.

What You’ll Do

As a Wellbeing Worker, you will:

  • Take patients home directly from hospital and ensure they are safely settled
  • Provide support for up to 6 weeks, helping people regain or retain independence
  • Use a solutions-focused approach to connect people with the help they need, whether through Age UK Lancashire or community-based services

What We’re Looking For

You will be a caring and empathetic individual, committed to helping people live independently. You should have:

  • A person-centred, flexible, and responsive approach
  • Strong decision-making skills and the ability to manage your own workload
  • Teamwork skills alongside the confidence to work independently
  • Excellent communication skills to build trust with clients, healthcare professionals, and colleagues
  • Good IT and digital skills, including Microsoft Outlook, Excel, and use of iPads or smartphones
  • The ability to manage a busy workload, maintain accurate records, and complete assessments
  • The ability to carry out light domestic duties and move around homes (often across two floors)
  • A full driving licence with business insurance, and willingness to use your own car to transport patients
