Role: Release and Transfer Nurse Coordinator
Location: HMP Wormwood Scrubs, London (Category B Prison)
Hours: Monday to Friday, 07:30 to 15:30
Salary: Up to £53,172 per annum (depending on experience)

About the Role
We have an exciting opportunity for a Release and Transfer Nurse Coordinator to join our Primary Care team at HMP Wormwood Scrubs. This is a rewarding role where you will deliver the highest standards of healthcare while supporting patients through their release or transfer, ensuring safe and effective continuity of care.

What You’ll Be Doing
As a Release and Transfer Nurse Coordinator, your responsibilities will include implementing and overseeing the Release and Transfer Pathway, managing patient health assessments, care planning, delivery, and evaluation including clinical risk assessments, running face to face clinics to support patients preparing for release or transfer, participating in multi disciplinary team discussions and working closely with healthcare colleagues, prison teams, and external organisations to ensure safe discharge planning, leading and line managing members of the Transfer and Release team, and collaborating with wider release teams in the prison and external agencies to support patients effectively.

Essential requirements:
NMC Registered Adult Nurse
Strong clinical expertise with evidence of applying local and national policies such as NICE guidance
Experience in developing care pathways, protocols, and monitoring outcomes
Previous line management experience

Desirable experience:
Previous work in hospital, community, or prison healthcare settings
