Role: Practice Nurse

About the Role
We are seeking a dedicated Practice Nurse to join our team. This role involves delivering high-quality patient care, supporting health promotion, and working collaboratively with colleagues to ensure effective healthcare provision. The Practice Nurse will be responsible for a wide range of clinical duties, with flexibility to adapt based on workload and staffing needs.
Core Responsibilities
- Act as or support the Infection Prevention Control (IPC) Lead, working in line with the IPC Policy through audits, training, compliance checks, and reporting.
 - Produce or contribute to the IPC annual statement with the practice manager.
 - Develop, implement, and oversee care management programmes for frail and housebound patients.
 - Assess patient needs and provide evidence-based healthcare tailored to individuals.
 - Create, implement, and embed health promotion and wellbeing programmes.
 - Implement and evaluate treatment plans for patients with chronic diseases.
 - Identify, manage, and support patients at risk of developing long-term conditions, reducing negative health outcomes.
 - Provide routine nursing care in accordance with evidence-based guidelines, NICE, and the NSF.
 - Diagnose and manage both acute and chronic conditions.
 - Prescribe medications within Patient Group Directions (PGDs) and own scope of practice.
 - Review patient medications to promote compliance.
 - Advise patients on prescribed and over-the-counter medications, including side effects and dosage guidance.
 - Coordinate the delivery of travel medicine services.
 - Request and process pathology services and results.
 - Oversee and deliver chronic disease clinics, referring patients to secondary or specialist care when required.
 - Maintain accurate and compliant clinical records.
 - Ensure effective use of read codes by all members of the nursing team.
 - Manage and maintain chronic disease registers.
 - Develop and deliver well woman and well man clinics.
 - Ensure compliance with the practice chaperone policy.
 - Assist GPs with minor surgical procedures.
 - Prioritise health issues and provide timely interventions.
 - Support the clinical team during emergencies.
 - Assess, recognise, and refer patients with mental health needs.
 - Deliver vaccination programmes for adults and children.
 - Support patients in the correct use of prescribed and over-the-counter medicines within scope of practice.
 - Liaise with external services and agencies to ensure appropriate support, particularly for vulnerable patients.
 - Delegate clinical responsibilities safely and appropriately.
 - Support the clinical team with safeguarding matters, following local and national policies.
 - Apply practice and local policies regarding substance abuse and addictive behaviours, ensuring appropriate referrals.
 - Provide opportunistic health promotion whenever appropriate.
